Be it Remembered,

personally and State appeared of Ohio, John abount
before me TOD B. Jydge and ex-officio
Clerk of the Probate Court, within and for the County of Franklin
and makes
report of himself for naturalization; who being duly sworn according to law, on his oath doth declare and say that it is bona fide his
INTENTION TO BECOME A CITIZEN OF THE UNITED STATES,
and to renounce forever all allegience State, or Sovereignty; and particularly to
and fidelity to any Foreion Princé, Republic Potentater
whose subject he is.
