In accordance with Executive Order 11222 and the regulations
governing the standards of conduct of White House staff
members (3 CFR Chapter V), all regular employees at a
level GS 13 (or equivalent) and above are required to file a
statement of employment and financial interests within 30
days after his entrance on duty. Additionally, each is
required to report any changes in this information at least
once annually, as of June 30, or to file a negative report
as of that date if no changes have occurred since his last
filing.
Pursuant to this requirement, you are requested to report
any changes in your previous statement by completing the
copy of the attached form and returning it to me, under
administratively confidential cover, by June 30. The other
copy should be retained in your personal files for future
reference.
In the event that for any reason you have not filed an original
statement since coming on the staff, you are requested to do
