I was born on the 25th day of august\nanno Domini 1 880 at Pico, agores Is.\nFourth. I emigrated to the United States from\nPico, agores\non or about the\n1st\nday of\n885 States, the port of Provinartown, Mass . on the 26th day of may\nmay\nanno Domini and arrived in the United at\nanno Domini 1895 on the vessel name forgotten (If the arrived\nglien otherwise than by vessel, the character of conveyance or name of transportation company should be given.)\nFifth I declared my intention to become a citizen of the United States on\nat\nSan diego California\nin the\nthe Superior 1st day of July Court of San Deego - Country anno -\nDomini 1909\n.\nSixth. I am m married. My wife's name is Maria of Souga azam\nShe was born\nin\nPico, agores\nand now resides\nat Point Loma\nI\nhave three children, and the name, date and place of birth, and place of residence of each of sald children is as follows: Odaura born\n(Give number, street, cityop town and State.)\nCalifornia\nat Mattapoisett, mass. I am not a disbeliever in or opposed to organized government or a member of or affiliated with any organization or body of persons teaching disbelief in or opposed\nto organized government. I am not a polygamist nor a believer in the practice of polygamy. I am attached to the principles of the Constitution of the United States, and it is my\nintention to become 9, citizen of the United States and to renource absolutely and forever all allegiance and Adelity to any foreign prince, potentate, state, or sovereignty, and\nparticularly\nto the state of Portugal\ncitizen\nof whom at this time I am a assbypet, and \" is my intention\nto reside permanently in the United States.\nEighth.
