I was born on the 23rd day of Way anno Domini 1 884 at Loughar, Wales Fourth. I emigrated to the United States from hiverpool England on or about the 18th day of august anno Domini 904 and arrived in the United States, at the port of New york on the 25th day of august anno Domini ,404 on the vessel " celtic" Fifth. I declared my intention to become a eitizen of the United States on the 6th (ii the allen arrived otherwise than by yesgel, the character of or name of transportation company should be given.) day of September anno Domini 1912 at San Diego, california in the Superior Court of Sixth. I am - Sarah E. (crull) Davies San Deigo County married. My wife's name is She was born in Illinois, u.s.a. , and now resides at 1753 w. Lewis St., San (Give Diego, California State.) number, street, city or town, and I have one and the name, date and place of birth, and place of residence of said childs is as follows: Frederick 9 apr 1913 bornand living al San Dizo, Cal Seventh. I am not a disbeliever in or opposed to organized government or a member of or affiliated with any organization or body of persons teaching disbelief in or opposed to organized government. I am not a polygamist nor a believer in the practice of polygamy. I am attached to the principles of the Constitution of the United States, and it is my intention to become a eitizen of the United States and to renounce absolutely and forever all allegiance and fidelity to any foreign prince, potentate, state, or sovereignty, and particularly to George V., King of Great Britain and Ireland of whom at this time I am a subject, and it is my intention to reside permanently in the United States.
