I emigrated to the United States from Liverpool, England on or about the 3rd December day of anno Domini 905 United the newyork 1. and arrived in the States, at port of , on the 14th day of December anno Domini 1905 on the vessel "Majestic (If the allen arrived otherwise than veasel, the Character of convoyance or name of Aransportation company should be given.) Fifth. I declared my intention to become a eitizen of the United States on the 31st day of December anno Domini 1909 at San Diego, California , in the Superior Court of San Diego county Sixth. I am not married. My wirt Sname is She was born in and now resides at (Glve number street, city town, and State.) I children, and tho-name, date and place of birth, and place of residence of such of said is as follows: Seventh. I am not a disbeliever in or opposed to organized government or a member of or affliated with any organization or body of persons teaching disbelief in or opposed to organized government. I am not a polygamist nor a believer in the practice of polygamy. I am attached to the principles of the Constitution of the United States, and it is my intention to become a eitizen of/ he United States and to renounce absolutely and forever all allegiance and fidelity to any foreign prince, potentate, state, or sovereignty, and particularly to Empeor faustia and of whom at this time I am a subject, and it is my intention to reside permanently in the United States.
