I was born on the 19th day of September anno Domini 1 889 at Swanton. Nebraska (maturalized Canada. 1911 in Fourth. I emigrated to the United States from Kingegatt B. C. Canada 10 th on or about the day of November Domini 1912 and arrived in the United States, at the port of Eastport. Idaho 10th anno on the day of November 10 1919 anno Domini 1912 on the wassel Can, Pac. I am not a disbeliever in or-opposed to organized government or a member of or affiliated with any organization or body of persons teaching disbellef in or opposed to organized government. I am not a polygamist nor a believer in the practice of polygamy. I am attached to the principles of the Constitution of the United States, and it is my nches, intention to become a citizen of the United States and to renounce absolutely and forever all allegiance and Adelity to any foreign prince, potentate, state, or sovereignty, and particularly to George V King of Great Britani and Ireland of whom at this time am a subject, and it is my intention to reside permanently in the United States.
