My occupation is Labover (Give number, street, city or town, and State.) California Third. I was born on the 27th day of December anno Domini 1 874 at Fourth. I emigrated to the United States from Rotterdam Holland London England on or about the - anno Domini 890 and arrived in the United States, at the port of day of October anno Domini 890 on the vessel newYork. n.y. on the - " Obdam day of october Fifth. I declared my intention to become a eitizen of the (If United the alien States arrived on otherwise the than by vesse), the character of conveyance, or name of transportation company should be given.) 4th San Diego. California day of at , Superior June in the anno Domini 1914 Sixth. I am not married. My wife's name is Court of balifornia : she was born on the day of at . anno Domini 1 and now resides at I have no children, and the name, date and place of birth, and place of residence of each of said children is (Give number, street, elty or town, and State.) as follows: to on e 125 Seventh. I am not a disbeliever in or opposed to organized government or a member of or affliated with any organtization or body of persons teaching disbelief in or intention to organized government. I am not a polygamist nor a believer in the practice of polygamy. I am attached to the principles of the Constitution of the United States, and it opposed is my particularly to to become a citizen of the United States and to renounce absolutely and forever gll allegiance and Adelity to any foreign prince, potentate, state, or sovereignty, and George V. King of Great Britain and Ireland to reside permanently in the United States. of whom at this time I am a subject, and it is my intention, Eighth. Iam able to speak the English language. Minth. I have resided October continuously in the United States of America for the term of Ave years at.
