I was born on the 12th December\nday of\nanno Domini 1890\nat Heredia\nbosta Rica\n918 M.\nFourth. I emigrated to the United States from\nnogales\nmexico\non or about the\n12th\nday of February\nanno Domini 1912 and arrived in the United States, at the port of nogales arizona\non the\n12th\nday of February\nanno Domini 1912 on the vessel\nSouthern Pacific Raileod.\n(If the allen arrived offerwise than by vessel, the character of convayance or name of transportation company should be given.)\nCourt\nFifth. I declared my intention to become a eitizen of the United States on the 18th day of September\nanno Domini 1916\nat\nSan Diego, balifornia\nin the\nSuperior\nCourt of California\nSixth. I am\nmarried. My wife's name is\nsadie\nshe yas born on the\n12th\nday,\nof august anno Domini 888\nat\nrears,\nColusa, California\nand now resides at 943-17th st. I am not a disbeliever in or opposed to organized government or a member of or affiliated with any organization or body of persons teaching disbellef in or opposed\nto organized government. I am not a polygamist nor a believer in the practice of polygamy. I am attached to the principles of the Constitution of the United States, and it is my\neside\nintention to become a eitizen of the Unitgd States and to renounce absolutely and forever all allegiance and Adelity to any foreign prince, potentate, state, or 'sovereignty, and\nparticularly to\nThe Republic of bosta Rica\nof whom at this time I am a subject, and it is my intention\nto reside permanently in the United States.\nRighth.
