Bedford, Mass. on August 16 1920 for permanent residence therein, and now resides at Point Loma, California have 5 children, and the name, date, and place of birth, and of are as place residence of each of sald children follows: Marie M. Now. 19, 1921 Lowell, Mass. fildred M. June 1923, Lowell Mass. Anibal M. July 1926 Leyell, lass.; Algerina M., May 26,1928, Lowell, Mass. John, Dec. San Diego, Calif: All living at San Diego, Califernia. (6) My last foreign residence was Faul do Max, Madeira. Portugal I emigrated to the United States of America from Junchal, Portugal My lawful entry for permanent residence in the United States was at New York, New York under the name of John Monas Freitas on July 1913 on the vessel Unknown as shown by the certificate of my arrival attached hereto. to organized government.
