July 13, 1918. New Amsterdam Casualty Co., #59 John Street, New York, N. Y. Gentlemen:- I am sending you, herewith, slips signed by Mr. Kunzman, in reference to the payment of partial disability. In reference to his operation, I am hand- ing you herewith, a statement made by Dr. Dowling of Orange, as to the charges for this operation. Kindly advise at the earliest moment possible as to what you will do in this matter. The hospital will be either St. Marys or the Memorial. Very truly yours, Superintendent.
