Dear Bir: I have this day examined for your company the above named case. Statement Nettie Madlinger; age 32 years;height 66 inches;weight 140 lbs; of Claimant Born in the United States; married; present residence, 302 Orange Street, Newark, N. Housewife and employed by the U.S.Radium Corp., Claimant began working with the U.S. .Radium Corporation in 1916, Claimant continued working steadily until she was ak laid of from her work in June 1921; Claimant returned again to work in September 1921 and continued working until December 5,1921, when she discontinued further work with the U.S.Radium Corporation. Statement is made by claimant, that she has not been able to perform any manual labor since leaving the employ of the Corporation. During her employment with the U. 8. Radium Corp., her usual work extended through an eight hour day. Claimant has been married 12 years; No children have been born;No miscarriages have appeared. Claimant living during her entire married life with her husband. Menstruation began about the age of 12 years; was regular from its commence- ment. Menstruation was scant, lasing two days, soiling 2-3 napkins per day. There has been no vaginal discharge between menstuual periods. No abnormal symptom- otology bef re, during or after menstru 1 periods. Claimant is suffering from an enlargement of the thyroid gland, right lobe, and posterior isthmus. Statement is made by claimant, that she had none of the usual eruptive diseases of childhood. No disease sinee grown to adult life. Examination made in the office of Dr. .W.Maver, 532 Bergen Ave. Jersey City, N.J. in the presence of Dr. Maver and Dr Neff, with whom treatment was begun January 15th, 192 and has continued at various intervals, up to and including the present time; Claimant visiting the office of Dr. Neff, usually about three times weekly. Reflexes:-Superficial and deep normal. Eyes:-Pupilsequal, - react to light and accomodation. Byegrounds normal. Field of vision normal. Movements of eyeball, S OW synchronous action in all of thoir movements. No horizontal or vertical nystagmus. No objective clinical evidence of disease of eyes, ey balls, nuscles or innervation thereof. Tests for refractive errors were not made. Ears:Hearing normal; Determined by watch held 2 inches from external auditory canal,also watch placed upon mastoid portion of temporal bone. Tuning forkvibrat on, distinguished with instrument placed upon vault of skull, chin, teeth, mastoid portion of temporal bone. Air conduction at distance, not made, because of no facilities in office. Canals partially filled with black wax, right and left, packed against drum membrane, inflammatory areola at periphery of said wax formationne Complaint of loss of hearing.
