Dr Bergners Report of Examination of OCT 24 1947 five girls on Oct 21st, 1947 Gladys Shea Age 49 Married. Family History. Mother died of stroke at 72 Father died accident at 37 Otherwise negative. Past History Med negative. Obst. Gravida V Para Oldest child 24, youngest 11 Menstr. 13/28 regular 3-4 days. Moderate flow. No dysmenorrhea. Menopause in 1937. Physical Examination. Well developed. Markedly obese Wt 260 lbs Ht 63 inches Head. Eyes- equal and regular, react to light and accommodation E 0 M normal ENT Vision without glasses R 20/25 L 20/25 with glasses R 20/20 L 20/20 Teeth very carious. Tonsils hypertrophied. Neck Normal Chest Lungs clear to a and p. Heart not enlarged. Sounds of fair quality. No T R M A2 < P₂ accentuated, and slightly metallic in quality. RSR VR= PR:80 Blood pressure 170/106 Extremities normal Impression: Essential hypertension, obesity, hypertrophied tonsils, carious teeth. Is acceptable in spite of these defects. Mary Hannaford Age 30 Single. Family History. Brother has rheumatic fever, otherwise negative. Past History Medical negative. Surgical Appendectomy at 21 T and A at 10 ears Menstr. 13/28 regular 2½ days. Moderate flow. Moderate backache. 1st day. Physical Examination. Well developed, well nourished. Wt 104 lbs Ht 56 inches. Head. Eyes pupil equal and regular. React to light and accom/ EOM normal Vision R 20/20 L 20/20 ENT normal Chest. Lungs clear to a and p. Heart not enlarged. Sounds of good quality. No TR or M A2 P₂ RSR VR= = PR = 7-2 Blood pressure 110/68 Extremities normal. Impression No defects. Margaret Laundry. Age 52 Widow. Family History Mother died 78 cause unknown Father died 47 pneumonia Otherwise negative. Past history. Med negative Surgical Hysterectomy and bilateral cophorectomy at 29. Chbbecystectomy at 44 Haemorrhoidectomy at 51 Memtr. 14/28 regular 2 to 3 days Moderate flow No dysmenorrhea Artificial menopause at 29. Obstetr. Gravida + Para 0 Miscarriage at 3 months Physical examination Well developed, well nourished. Wt 171 lbs Heighe 65 Head. Eyes Pupils equal and active react to light and accommod. EOM normal Vision R 20/20 L 20/20 ENT normal. Neck Normal Chest Lungs clear to a and p Heart not enlarged. Sounds of good quality. No T or R. There is a harsh blowing systolic murmur at apex, not transmitted.
