You have been scheduled for your blood count and
expired air examination on Friday, November 26th. You are to
report at 9:00 A.M. to:

15 Amsterdam Av. (2nd Floor, Room 32),
New York City
Above location is at 60th Street and Amsterdam Avenue. Take 8th
Avenue Subway to 59th Street and walk two blocks toward river.
Wash your hair the night before the examination,
bathe and wear all clean clothes. Also bring with you a specimen
of urine, collected early A.M. of day of examination.
If, for any reason, you cannot go, please notify us
immediately so that other arrangements can be made.
