UNITED STATES RADIUM CORPORATION The undersigned, being the registered owner of the number of shares hereinbelow set opposite his use of the 87 Cumulati Ve Preference Stock of United States Redium Corporation (the name of such stock having been changed to the above name from its former name 07 Cumuistive Prior Preference Stock, by amendment filed with the Secretary of State of Delaware on June 11, 1929, which said stock 988 created by emendment filed with the Secretary of State of Delaware on March 5, 1929, by reclass fication, share for share, of the then existing convertible seven per cent. cusulative preferred stock of said Corporation), pursuant to & certain plan for the readjustment of the capital structure of the Corporation, duly adopted by the stockholders of the Corporation at a special meeting hold on June 4, 1929 and adjourned to June 10,1929, hereby surrenders to said Corporation the certificates representing such shares (such certificates being certificates for the said convertible seven per cent. cumulative preferred stock) and does hereby release and surrender all rights to any and all unpaid custintive dividends now accrued or which any heresfter accrue on said $7 Cusulative Preference Stock or on seid 07 Cumulative Prior Preference Stock and on all shares of said convertible seven per cent. cumulative preferred stock which UGB reclassi- fied into the sene as aforessid, and also on any and all other shares of any of such stock which may be owned by the undersigned: Name of Stockholder Number of Shares
