The one addition that we had not previously discussed is the prohibition on the use of federal dollars to fund needle "distribution" programs as opposed to needle "exchange" programs. This was included to appease some of our more conservative friends who fear that outright distribution as opposed to one on one exchange might increase the use of drugs. While the AIDS groups will be less than happy about it, the fact is that the biggest and most successful needle exchange programs in the country are "exchange only" programs. This looks good. Keep your fingers crossed they are in conference now. FYI The situation in Chautaugua County, NY continues to generate a tremendous amount of interest. Fortunately, this is an example of public health, law enforcement and the judicial system effectively working together in the interest of their citizens. Unfortunately, their best efforts could not prevent this tragedy. This seems to fit nicely into overarching efforts on the part of the Administration to support children, youth, and families. Our efforts on behalf of young children have been quite high profile lately and I am pleased that Mrs. Clinton will spend some of World AIDS Day with the Pediatric AIDS Foundation. In addition, I think an emphasis on youth is also important, reinforcing the myriad of things the Administration is doing to support young people in their successful transition to productive adulthood. To steer a steady course, all young people need support and guidance from caring adults in their families, their schools, and their communities. They need opportunities and experiences that promote positive development. And they need a sense of hope and possibilities. They need to believe they have a future worth working for if they are to be motivated to stay in school, resist negative peer pressure, and make responsibile choices. It is in this context that our HIV prevention challenge must be viewed. The tragedy in Mayville, NY provides us with a unique opportunity to refocus the nation on the importance of HIV prevention and youth. I would propose that the President host a National Town Hall meeting calling for parents, teachers, coaches, counselors, YMCAs, Boys Club, etc. to join together in supporting our young people. HIV prevention should not be a wholely separate effort but an integral part of Administration efforts to support schools, national service, volunteerism through the Presidential Summit, after school programs as part of the new child care initiative, etc. To bring visibility of this issue, the President could gather an array of interested parties in a school gym/auditorium, or on MTV, for a town meeting. He could be joined on stage by others who could make brief presentations on model efforts, discuss the important of action, and help in answering pre-arranged questions. Such individuals could include: Magic Johnson, Surgeon General Koop, the new President of the PTA (a friend of the VPs from TN), a teacher/counselor, an HIV(+) young person, and me. A few model efforts clould be highlighted such as: a national service volunteer doing AIDS education, a member of the "Dream Team" high school basketball players doing AIDS education, and a kid from the "Safe Passage" program established by Arthur Ashe for inner city youth. This discussion could be downlinked into other World AIDS Day gatherings for young people across the country. A "no new money" initiative could be announced to re-direct some additional CDC HIV prevention money to peer education and to better integrate HIV prevention existing on AIDS programs targeted at youth. The intent, I believe, is to leave it up to the authorizers to handle. Please bear in mind that the conference committee has not completed its work, and it ain't over 'til it's over. There may also be additional report language added or current language may be changed. Given the response to what happened in New York with the Nushawn Williams case, and the fact that over 50% of new HIV infections occur in people under 25, I think we would do well to demonstrate leadership on this issue at this time. It is consistant with the high prioritiy we have placed on education and young people. In addition, I just received the latest figures on the epidemic worldwide which reveal that there were 5.7 million new infections last year, as opposed to the 3 million we had predicted. That means that the epedemic is spreading nearly twice as fast as we thought. This is not good news, and has devestating consequences for children and youth worldwide. We are at an incredibley critical point in this epidemic. The leadership of the United States at this juncture will literally determine the course of the epidemic in the years to come. We are all in this together and we must all be part of the fight. In the 15 years of this epidemic 362,000 Americans have died many of whom got infected when they were your age. More and more HIV, the virus that casues AIDS, is affecting young people just like you. Today in the United States half of all new HIV infections are among people under the age of 25 -- and 1 out of every 4 is a teenager. In fact, every hour, more than two young people are infected. In some communities, as many as 1 in 30 18 and 19 year old males has HIV. In truth, AIDS is not always as far away as you may think. We know that this virus does not discriminate. HIV doesný, care who you are, what you wear or how good you look. (FYI-- young people generally say the people they hang out with look too good to have HIV). It doesn1, t care if you are male or female, gay or straight, rich or poor, black, white, yellow, brown or red. If you mess with the virus it will mess with you. * Knowledge is power and education is the key. That0, why each and every one of you needs to empower yourself for a healthy and successful future. This means that the expected 40 million people by 2000 is quite low. That being the case, demonstrating a US leadership role in the global epidemic is very important. TALKING POINTS ON MEDICAID EXPANSION TO PEOPLE WITH HIV Background In April, the Vice President directed the Health Care Financing Administration (HCFA) of HHS to determine the feasibility of establishing a demonstration program to expand Medicaid coverage to people who were HIV -- and do not currently qualify for Medicaid benefits. This was in response to a release by HHS of a standard of care for HIV and AIDS that recommended early treatment. The Vice President asked HHS to report back within 30 days. Under current regulations, Medicaid coverage is not availalable to many individuals with HIV until they progress to AIDS; the new treatments offer the promise of forestalling the progression to AIDS, creating a "catch 22" whereby individuals can't get the drugs that would keep them from progressing to AIDS until they get AIDS. Their market research indicates a real desire for basic information from a "credible but non threatening source" ( I am not sure I fit either criteria! ). While I don't exactly have tons of time on my hands, a little project like this might well be a fun and creative diversion. Doing a little prevention work myself couldn't hurt , and could be good PR for the office. Of course, any pennies in profit would go to an AIDS fund ( or to the AIDS office fund only kidding! ).
