The show will air this Sunday morning as well as Sunday evening and will also be replayed Monday morning. The point of this show is to give you the opportunity to explain the health care issue in simple terms. You will be asked to explain the Mitchell bill and what it will mean for people who are currently satisfied with their insurance, for people with pre-existing conditions and for people with no insurance. He may ask you what you have learned over the past 18 months and to look back on the process. This should be a straight forward interview. Remember that the best way to simplify this issue is to communicate to the viewers through stories. I have attached background information for you to review. NOTE PREPACED FOR VPOTUS MESSAGE DOCUMENT ADMINISTRATION COMMITMENT TO FIGHTING FOR UNIVERSAL COVERAGE President Clinton yesterday put forth a call to action to his Cabinet. This Administration will devote its every resource to fighting for guaranteed private insurance that can never be taken away. * The Cabinet will be involved in an unprecedented level of activity. They will be doing regional media every day - print, radio and satellite tv interviews, will meet with groups and meet with members of Congress. Cabinet has already completed over 90 editorial board meetings. * President, First Lady, Mrs. Gore and myself have and will continue to deliver the message directly to the American people that we are going to be working night and day to achieve universal coverage for hard working middle class Americans. We'll be participating in public events, meeting with groups and make ourselves available to the media as we are doing today. * President will be speaking directly to the American people during the coming days on CNN to help them cut through the misinformation and confusion and understand what health care reform will really mean for American families - they will have health care security, they can keep their doctor and their plan and health care quality will be maintained. WHY UNIVERSAL COVERAGE? * We have made extraordinary progress and stand ready for an historic debate.. The Senate and House are now poised to cast an historic vote in favor of hard working middle class Americans. We have come this far and we cannot turn back. * Debate is going to be over whether we should guarantee every American health care coverage that can never be taken away or whether we leave millions of middle class Americans out in the cold. * Without universal coverage we: * cover the poor, cover the rich, but leave middle class families without real health security do little to control costs The President's second nightly address to the American people on health care will be broadcast tonight on CNN at 7:50 p.m. Eastern time. The topic of tonight's Oval Office address will be choice of doctor and health plan. CHOICE OF HEALTH PLAN AND DOCTOR Universal Reform Will Mean More Choice Under the House and Senate bills, all Americans will have a choice of insurance plan and doctor. People will be able to shop for the best coverage at the best rate - because there will be more competition in the marketplace. People who are happy with their current insurance plans and doctor can keep them. Universal reform will protect the doctor-patient relationship. That's one reason why groups such as the American Medical Association, the American Nurses Association, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Hospital Association - the nation's leading provider groups -- support a universal approach. Today, more and more Americans are being forced into insurance plans where they are separated from their doctor. And most Americans have little or no choice of insurance plan. The Republican Alternatives Offer Some Unattractive Choices Under the Dole plan, a family of four that makes $30,000 has the choice of spending nearly $6,000 -- 19% of their income - for health insurance or having no insurance at all. (National Journal, 7/16/94) The insurance companies can choose to charge older workers as much as four times more than younger workers. Employers can choose to drop employees' coverage or cut back benefits. QUESTIONS AND ANSWERS ON ADS O: Why paid ads on CNN? Why not just a prime-time Oval Office address on the networks? A: The White House asked ABC, NBC, CBS and Fox if they would broadcast an address by the President to the American people on this critical issue of health care. They all said no. The DNC then asked to pay for time on these networks to broadcast a critical message on health care from the President. The networks said no. But the President strongly believes that the American people must have the facts before Congress votes on this historic decision. That is why we decided to take this unprecedented step in paying for a series of broadcasts on CNN. [So far in 1994, the President has made no Oval Office addresses. In contrast, in the second year of his first term, President Reagan was able to make 6 Oval Office addresses.] a: Isn't this just a last minute attempt to save a dying bill? A: Not at all. Studies show that $50 million has already been spent to influence this decision - most of it by opponents of reform. And millions more will be spent in the coming weeks. The President wants to take his case directly to the American people before this historic vote. a: What is the President selling? Is it Mitchell? Is it Gephardt? A: The President will be talking about key elements of reform that are contained in both approaches. And he will be talking about the importance of universal coverage - the central element in his original proposal, the House bill and the Senate bill. So while the opponents of reform are trying to raise fears about this, they are trying to tell you that you could lose what you now have, that things could get worse under reform -- The fact is at a minimum you keep the health insurance you already have - Except that under thi I new approach, if you lose your job, you won't lose that health insurance; It won't be canseled if you get sick -- And your coverage can never run out or be cut off. And because you'll have more cho:ces, and will be able to shop for the best insurance at the best price, there will be more competition in the marketplace - and that will finally put the brakes on the skyrocketing COSIS of health care. All of these elements are essential parts of an approach that will give you better health care, at a better cost .. private health insurance tha. can never be taken away. Thank you and I will talk to you again tomorrow night. Talking Points - August 4, 1994 The President said yesterday that he would sign the Mitchell bill. One of the Administration's biggest health care reform supporters, the AFL-CIO, has said it cannot support the Mitchell bill and that the bill should be strengthened to assure universal coverage. The AFL-CIO supports the Gephardt bill and so does the President. The President is fighting for universal coverage and he believes that bills unveiled by Gephardt and Mitchell achieve universal coverage and protect the health security of working middle class Americans. Won't this hurt chances of passing health care reform in the Senate? The AFL-CIO wants the Senate bill to be strengthened and they will be working to make this happen. They are committed to achieving universal coverage and that's a shared goal. Did the AFL-CIO bring this up at the meeting with the Chief of Staff yesterday? I don't know the details of the meeting. My understanding is that they discussed their commitment to fight for universal coverage and real protection for working Americans. What about Senator Wellstone and others saying the Mitchell bill does not achieve universal coverage? My understanding is that these Senators said that the Mitchell bill needs to be strengthened. They also said that 95% is not universal. We agree and Senator Mitchell agrees that 95% is not universal coverage and that's why he put mechanisms in his bill to go beyond and reach universal coverage. Does the Administration think that the Mitchell bill needs to be strengthened? The Administration wants universal coverage. That's our bottom line. We believe that the Mitchell bill achieves universal coverage. If the Senate strengthens the Mitchell bill, that's fine. As the President said last night, he is not going to get into being a legislator. Congress will make changes, that's part of the legislative process. Our bottom line is universal coverage. Senator Dole says that his plan solves the problems of people like those at the press conference last night. Why not just enact the Dole plan? That's just not true. As we've said all along, you can't solve the problems of portability or pre-existing conditions without universal coverage. Unless coverage is universal, you'll still have people like Mr. Cox, whose wife got care too late because they weren't covered, and who had care denied because she had a pre-existing condition. The President said last night he wants a plan that will work for Mr. Cox. and that means universal coverage. Senator Dole has said that there are 17 new taxes in the Mitchell bill. Secretary Shalala today said that there are not. How can the Administration call this bill an improvement if there are 17 new taxes? There are not 17 new taxes. Instead of issuing attacks, why don't they introduce their own bill? They put out a proposal weeks ago and they still don't have any legislation. It is easier for them to attack than to be constructive. Hot Button Health Care Qs and As - August 2, 1994 What happens if the Mitchell bill does not achieve 95% If 95% is not reached by 2000. employers and employees will split the cost of insurance premiums starting in 2002, unless Congress approves an alternative recommendation developed by a national commission to bring us to universal coverage. What happens if 95% coverage is achieved by 2000, would that be acceptable? I want a guarantee in law that every American have health care coverage that can never be taken away. Senator Mitchell has put us on the road to achieving this goal. If 95% is reached, a commission would issue recommendations to Congress to bring us the rest of the way to achieve universal coverage. Let me be clear: my bottom line has always been and remains: universal coverage guaranteed health insurance that can never be taken away. But Congress could reject these recommendations. Wouldn't that mean that we could end up with 95% of Americans covered? I think that members of Congress will want to make sure that hard working middle class Americans have real health security and they will move forward to make this happen. Is 95% universal coverage? As Senator Mitchell said 95% is not universal coverage. That is why he put mechanisms in his bill to move beyond 95%. Would you sign the bill put forth by Majority Leader Mitchell? The bills put forth by the House and the Senate do protect the health security of hard working middle class Americans by achieving universal coverage. But to discuss signing bills that have recently been introduced and have yet to be debated let alone go to conference seems to be premature. Isn't the Medicare Part C provision in the House bill simply a large government-run program? Haven't you consistently said that you reject a large government-run program? The House leadership has chosen to adopt the method similar to that developed in the Ways and Means Committee bill as one of the ways to cover those who are not employed and as an option for small firms. It helps achieve universal coverage. and, like many other issues. will be the subject to continuing debate. Abortion is in both the House and Senate bills, but is not expected to pass as introduced. You had abortion coverage in your bill. Will you sign a bill without abortion coverage? Most plans cover abortions today. My bill included pregnancy-related services, including abortions. This is obviously a contentious issue that will play itself out on the House and Senate floors. But would you sign a bill without abortion coverage? Let's let the legislative process work. I believe all pregnancy-related services should be covered, just as they are today. In your State of the Union Address you said that you would veto any bill that does not guarantee universal coverage. Does this still hold true? I think Congress will deliver a bill that achieves my bottom line of universal coverage. It's what the American people want and what they deserve. Additional Health Care Qs and As - August 2, 1994 How do you define universal coverage? Is it 100%, 99%, 95% Universal coverage is a guarantee in law that every American have health care coverage that can never be taken away. It's what the overwhelming majority of the American people want and deserve. Is it 100%, 99%, 95%? It's a guarantee in law that every American have health care coverage that can never be taken away. That's the only way the system will work. The real question is what will happen if Congress fails to achieve universal coverage. Will middle class Americans be denied real health security and can health care costs be controlled. Many people are saying there just aren't the votes for an employer mandate. Can you get to universal without it? We continue to believe that shared responsibility between employers and employees is the best and fairest way to achieve universal coverage. It's the only way we know works. It's the way that's been working in the State of Hawaii for 20 years, and it's the way that works for most families now: health insurance on the job. You know, we hear a lot from the businesses who don't want this plan because they don't cover their workers today. But the fact is, most companies do provide insurance to their employees, and they are being forced to carry the extra burden for the people who now ride free. If we ask everyone to pay their fair share, we can make health care affordable for all businesses. Senator Mitchell's plan has a 50/50 split on the mandate. Would you accept such a mandate? I have said all along that I would be flexible on how to get to universal coverage, but not whether we get to universal coverage. Senator Mitchell has put forth legislation to bring us to universal coverage. Congressman Gephardt has put forth legislation to bring us to universal coverage. We have to see how the debate proceeds, but the bottom line is that it has to bring us to universal coverage. Do you support the exemption for small business being put forth by Senator Mitchell? The current health care system is stacked against small businesses. Many provide and those that don't want to. I have said all along that I want to make sure that small businesses are protected - so that they can provide insurance and keep small businesses thriving. But do you support such a concept? My bottom line is universal coverage. I'm flexible on how you get there, but not whether you get there. Do you have the votes for a mandate in the Senate? It will be close, but we will prevail Do you have the votes now? The bill was just introduced. It will be close. but we will prevail. The American people want universal coverage. They understand that without it middle class Americans will not have real health security. I think when the House and Senate engage in the health care debate on the floor, they will see that what we are talking about is whether they turn their backs on hard working middle class Americans or not. I believe the American people will be engaged in this debate and will be sending a strong message to their members of Congress that nothing less than universal coverage will be acceptable. Isn't it true that the House is afraid of being BTUed? If the mandate fails in the Senate, won't that bring it down in the House as well? I think the Senate and House will both pass a bill with universal coverage. I understand the concept of being BTUed, but I think members of Congress understand that a vote for universal coverage is a vote for the people that they represent. It's a vote for middle class Americans who are one job loss or one illness away from losing their health care coverage. It seems from this morning's papers that there is tension in the House given what is being introduced in the Senate. Won't this tension lead to a more moderate bill like that being proposed by Cooper? This is a complicated issue and members of both Houses are trying to grasp all of the details of the two bills being put forth. I think as the debate proceeds and members focus on the fight for health security for their constituent, for hard working middle class Americans, the answer will be clear: we need universal coverage. The Annenberg Public Policy Center said yesterday that the White House elevated the effectiveness of the Harry and Louise ads. Do you think the White House was mistaken in taking on Harry and Louise? There has been tens of millions of dollars spent against the fight to protect the health security of hard working middle class Americans. Obviously, this kind of money is going to have an impact on the public. But the bottom line is that the American people are committed to the President's bottom line of universal coverage. And that's what we're fighting for. TALKING POINTS: THE LEADERSHIP BILLS ACHIEVE PRESIDENT'S GOAL OF UNIVERSAL COVERAGE: Both the House and Senate bills achieve what the American people want -- Universal Coverage -- guaranteed health insurance that can never be taken away. The bills ensure real health security for hard working middle class Americans who deserve nothing less. House Bill: By 1999, all Americans will be covered. Employers will be asked to contribute 80% of the cost of insurance for their employees. Senate Bill: Should these voluntary efforts fail to cover 95% of the American people by 2000, employers and employees will split the cost of insurance premiums evenly starting in 2002, unless Congress approves an alternative recommendation that achieves universal coverage developed by a national commission. Businesses with fewer than 25 employees would be exempt. If more than 95% are covered, the Commission will recommend how to cover those without insurance. ELIMINATE ABUSIVE INSURANCE COMPANY PRACTICES: The proposals embrace the consensus insurance reforms that enjoy overwhelming support in the Congress. They outlaw preexisting condition exclusions, lifetime limits, and the practice of dropping coverage or raising your rates if you get sick. They ensure portability so that if you switch jobs or lose your job you can take your coverage with you. Senate bill: Unlike bills which rely on incremental reforms alone, the Senate bill is designed to avoid any negative consequences of market reforms during the transition period, by incorporating strong protections that guard against premium increases and by specifying a date certain by which all Americans will be covered. FISCALLY SOUND WITH ADDED PROTECTION FOR FEDERAL BUDGET: To provide ironclad protection to the federal budget, the plans contain fail-safe mechanisms to ensure that the cost of reform does not exceed the savings and revenues. FINANCIAL SAFEGUARDS FOR FAMILIES AND BUSINESSES: The proposals foster competition and harness market forces to keep costs down. Both plans call for careful monitoring of private health care costs and put the burden on Congress to take action if market competition fails to control costs adequately to protect American families and businesses. ALLOW PEOPLE TO KEEP WHAT THEY HAVE WHILE PROVIDING MORE CHOICES: People will be able to keep their current insurance plans and doctors that they are satisfied with today. In addition, through the creation of voluntary purchasing pools and the opportunity for private individuals to join the Federal Employees Health Benefits Program, the plans give people new coverage options with more choices of plans. House Bill: The House bill also creates a new federal insurance program, Medicare Part C, to provide even greater choices for coverage. PRESERVE AND STRENGTHEN COVERAGE FOR SENIORS: Medicare is preserved and it will be expanded to include coverage for outpatient prescription drugs. In addition, the House and Senate plans establish a new home and community-based long-term care program to give older Americans and those with disabilities additional options for care. ENSURES HIGH-QUALITY COVERAGE AND CARE: The benefits package in both bills emphasize preventive care and include coverage for prescription drugs to help keep people healthy, not just to treat them once they become sick. A portion of each premium will be earmarked for medical research to encourage the technological advancements and improvements that have made American medicine the finest in the world. PROVIDES GREATER PROTECTION FOR SMALL BUSINESSES Both bills give special protection to small businesses and enable small businesses to join purchasing cooperatives so they can get affordable coverage. House Bill: The House bill provides discounts to small low-wage firms through a tax credit; phases in the requirement to cover their workers; firms of 100 or fewer employees are not required to contribute until 1999. Senate Bill: Exempts firms with fewer than 25 employees from the back-up employer requirement to ensure universal coverage MAKE CHILDREN OUR NUMBER ONE COVERAGE PRIORITY: Both proposals makes sure that the 9 million children in the United States who are uninsured will now be covered. House Bill: Covers all Americans by 1999. Senate Bill: Targets funds to ensure that coverage is extended to as many uninsured children as possible right from the start. Investing in these children will ensure that they get off to a good start and come to school ready to learn. The Mitchell "You're Covered" Plan If you're looking for real insurance you can count on...you're covered. Unlike the Republican alternatives, the Mitchell bill has a simple purpose -- real, honest insurance that you can depend on. It will guarantee that all Americans have secure, affordable insurance they can depend on. It sets up a clear path to reach universal coverage -- ensuring hardworking middle class families that their health care will always be there for them no matter how their personal circumstances may change. In stark contrast to Republican proposals, the Mitchell bill guarantees real insurance without the fine print. By banning fine print and deceptive practices -- guaranteeing that insurance companies can't drop your coverage, eliminating lifetime limits, limiting pre-existing condition exclusions and making sure they can't jack up your rates -- the Mitchell plan guarantees that your health insurance won't cut out or disappear when you need it most. If you're looking for health care you can afford, you're covered. The Mitchell bill provides help to working families struggling to make ends meet -- so those families can finally afford health insurance. It prevents the insurance companies from jacking up your rates when you get sick. And it stops them from charging older people four times as much as younger people. This guarantees that people have affordable insurance they can depend on. If you're a child with no insurance. you're covered. Under the Mitchell bill, covering the 9 million children without insurance is the number one coverage priority. By providing generous financial assistance toward private insurance for children, the Mitchell bill guarantees affordable coverage to middle-class and low-income working families. If you lose your job...you're covered. The Mitchell bill guarantees affordable coverage for people who lose their jobs. When people are in between jobs and under the financial strain of being without a paycheck, they won't face the additional burden of bearing the entire cost of their premiums. Unlike incremental alternatives, your insurance will be secure -- something you can count on. If you're trying to get off welfare...get a job and you're covered. For working people who might otherwise have difficulty affording insurance, the Mitchell bill provides financial help to guarantee affordable coverage. If you move off welfare, and take a job, you'll be protected. If you're a small business you're covered The Mitchell bill helps small businesses get affordable coverage for their families and employees. It outlaws the insurance company practice of "redlining" -- where they refuse to cover certain small businesses just because they're not big enough or because an employee has a pre-existing condition. The Mitchell "Real Protection From the Insurance Companies" Plan Bans Fine Print and Deceptive Practices The Mitchell bill outlaws the most abusive insurance company practices -- making sure you have real, honest insurance you can depend on. It prohibits insurance companies from dropping your coverage when you get sick or refusing to cover serious illnesses. And it eliminates lifetime limits -- the fine print that the insurance companies now use to cancel your insurance when they don't want to pay the bills. This proposal guarantees that your health care won't cut out or disappear just when you need it most. The bill also says to insurance companies: there are certain benefits which you must cover -- like preventive care, or cancer treatment. So insurance companies can't decide what to cover and when to cover it. They will no longer be calling the shots. They will no longer be in charge. They cannot take away your coverage. Puts Consumers and Small Businesses in the Driver's Seat The Mitchell bill creates purchasing pools to give individuals and smaller businesses (those with fewer than 500 employees) bargaining power to get high quality coverage at affordable rates. Small businesses and individuals will be able to get coverage at the same rates as the big guys. The insurance companies will no longer have all the cards. They will no longer be able to pick and choose who to cover. Insurance companies will be forced to compete for your business -- and you will have a choice of insurance plans. Protects Small Businesses From the Insurance Companies Small businesses will no longer be forced to pay 35% more than big businesses for the same insurance. And the insurance companies will no longer be allowed to drop coverage for small businesses when an employee of that business gets sick -- or for any other reason. Protects Older Americans From the Insurance Companies In today's system -- and under half-measures -- insurance companies can charge older people as much as four times as much as younger people. That will be illegal under the Mitchell bill. Taxes Insurance Companies to Help Pay For Coverage The Mitchell plan protects people from insurance companies that try to jack up their rates and charge too much. Under the plan, insurance companies with skyrocketing rates will be taxed 25 percent on the difference between actual premium increases and a fair rate of return. According to CBO, the Mitchell bill pays for itself and the new coverage it achieves in part with $146 billion from insurance companies. The Mitchell "Good For Older Americans" Plan Makes Insurance Real For Senior Citizens In today's system -- and under the Republican alternatives, insurance for seniors often doesn't even meet their most basic needs. The Mitchell proposal makes insurance real for senior citizens by extending coverage to critical areas like prescription drugs and long-term care. Provides Prescription Drug and Long-Term Care Benefits to Seniors Under the Mitchell plan, Medicare will be preserved and strengthened. For the first time, Medicare will cover the cost of out-patient prescription drugs, the leading out-of-pocket health care cost for seniors. Older Americans will no longer be forced to choose between food and medicine. The Mitchell plan includes important new long-term care initiatives to begin to address this financially devastating problem for seniors and those with disabilities. It helps seniors get the home and community-based care they need and helps cover the cost of extended nursing home care. AARP Has Endorsed the Mitchell Bill These important benefits of prescription drugs and long-term care are among the reasons that the American Association of Retired Persons (AARP) recently endorsed the Mitchell proposal. The AARP said that the Mitchell bill provides an "historic opportunity to provide each of us with affordable, high-quality health and long-term care." Limits How Much Insurance Companies Can Charge Seniors In today's system -- and under haf-measures -- insurance companies can charge older people as much as four times as much as younger people. That will be illegal under the Mitchell bill. So older Americans -- not just young, healthy Americans -- will have real, honest insurance that you can depend on. Seniors Will Still Have Choice of Doctor Medicare will be protected, and older Americans will be able to choose their doctor /-- just as they do today. People will be able to keep their doctor and receive all the benefits they get from Medicare today. The Bob Dole "You're Out Of Luck" Plan If you're looking for health insurance you can count on you're out of luck. Unlike Senator Mitchell's plan. which guarantees secure. affordable insurance. the Dole plan provides no guarantee of decent coverage. Millions would still have phony, fly-by-night insurance, and an estimated 30 million Americans would have no coverage at all [Lewin-VHI, July 1994] If you're looking for health insurance you can afford you're out of luck. The Dole plan allows insurance companies to continue to raise rates higher and higher each year, and to charge older people three to four times more than younger people. Some small businesses will continue to pay more than others, and some families more than other families. You could still work hard, pay your premiums, and have medical bills sent back "not covered". As his Republican Colleague John Danforth said of the Dole plan. "It creates a new entitlement and it doesn't have any cost control I don't think we can do that. "[AP, 8/10/94] Newsweek predicts the Dole plan "will increase premiums for middle class people and could increase [the] number of uninsured.' [Newsweek, July 25. p. 19] If you're a senior you're out of luck. The Dole bill takes significant money out of Medicare and does little or nothing for seniors - no prescription drug coverage, and pitiful help with long-term care. If you're a child with no insurance you're out of luck The Dole plan will continue to leave an estimated 30 million people with no coverage, including 6.2 million children. What does that mean? In means millions of kids will go without seeing the doctor, millions will not get needed health care in time to prevent disabling illnesses. And under Dole's plan, even workers who get coverage can find that their insurance policy covers them, but not their kids. Employers will continue to drop back family coverage and offer bare-bones, worker-only policies -- leaving millions more children at risk of losing the coverage they have now. If you're a small business you're out of luck The Dole bill continues to permit insurers to charge higher rates to small employers just because they're small [Dole Bill 9002(d)(1) p. 117] Small companies pay 35% more on average as it is, and this insurance company abuse against smaller employers will mean the little guy still gets overcharged. And unlike the Mitchell proposal, there are no subsidies to help small businesses who can't afford coverage today afford it tomorrow. If you lose your job you're out of luck The Dole plan theoretically allows people to take their same insurance with them when they leave their job and go to a new one (portability), this only helps those who can pay the full premium themselves. That is not realistic for most people, since they can't afford the full cost of coverage, especially if they are without a job. In fact, most workers have that protection now, either through state insurance laws or through COBRA coverage. But as a recent study by the Department of Labor shows, only one in five can take advantage of it -- the rest can't afford it. Even Senator Chafee admits:' have great trouble seeing how you get portability without universal coverage." The Bob Dole "The Insurance Company Protection" Plan Insurance Companies Can Still Deny Coverage Through Loopholes and Fine Print While the Dole plan describes a standard benefits package (called the FedMed package) and says that every health plan has to offer that option. it also allows insurance companies to offer any benefits package they want. [Dole Bill Sec. 21115 (a) p.85 I This allows insurance companies to effectively deny coverage of certain illnesses by structuring benefits packages to not cover certain treatments. Millions of Americans will continue to-face the nightmare of insurance claims coming back: "Sorry, not covered." Few People Will Benefit From the Community Rating Reforms Many of the insurance reforms in the Dole bill, modified community rating for example, apply only to the "community rated market" but there is no "community" in the bill's reform. Any employer can self-insure, so employers with young, healthy employees will likely stay out of the community-rated pool and provide insurance on their own. In addition. associations can opt out of the community-rated pool and buy as a group, leaving even fewer people in the pool. This "vicious spiral" will lead to very high premiums in the community-rated market and will encourage healthier people to drop coverage. Mid-Size Businesses Could Face Large Premium Increases Under the Dole Bill, many small to medium-sized companies would be left between a rock and a hard place -- too small to self-insure, but too large to get the benefits of insurance reform in the community- rated pool. An employer with 55 employees could have extremely high costs because of just one employee with a history of illness. They could see their rates jacked up based on one worker with a serious medical condition. For most firms in this category, insurance costs will continue to be high and unpredictable. Insurance Companies Can Decide They Don't Want to Sell to Small Businesses Insurance companies can decide that they want to avoid the small business sector altogether and refuse to sell insurance to any small businesses. Under the Dole plan, they could continue to deny coverage to small businesses. Insurance Company Profits Protected, Middle Class Families Left Waiting The Dole plan limits, but does not eliminate. the ability of insurance companies to deny or forestall coverage for "pre-existing conditions" [for six months to a year]. While all agree that pre-existing condition limitations are a necessary transition tool to universal coverage, the Dole bill will never get there, so exclusions will never go away. Insurance Companies Could Charge You Three to Four Times More Because You're Older The Dole plan allows insurance companies to charge older workers three to four times more for insurance than younger workers, and twice as much as under Senator Mitchell's bill. Americans with Insurance Would Still Be At Risk of Losing It The real bottomline is that under the Dole plan. everyone remains at risk of losing the insurance they have now -- because under a non-universal system, no one is guaranteed protection. As Newsweek magazine reported last week, the Dole plan "will increase premiums for middle class people and could increase [the] number of uninsured." [Newsweek, July 25, p. 19] The Bob Dole "Cheating Seniors" Plan Raids Medicare and Gives Seniors Nothing Back While other bills reduce the rate of growth in Medicare. the Dole Bill relies heavily on savings from the Medicare program to finance reform. The Dole bill gives nothing back to the older Americans that Medicare was set up to serve. No help with prescription drugs. Nothing but lip service on long-term care. Forces Millions of Seniors to Choose Between Food and Medicine The Dole plan does not add prescription drug coverage to Medicare -- giving millions of older Americans no help paying for costly prescriptions. Prescription drug costs are the highest out-of- pocket expense for three out of four seniors, and the Dole plan would provide no help, forcing millions of older Americans to continue to choose between food and medicine. More than 31 million Americans under age 65, and 18.5 million Americans over age 65 would be denied prescription drug coverage under the Dole bill. [NCSC, "Six Reasons the Dole Bill is Bad for Seniors," 7/94] That's just one reason the AARP calls the Dole bill "a harmful prescription for òlder Americans" Leaves Older Americans in Need of Long-Term Care at Grave Risk Unlike Senator Mitchell's proposal. which invests $50 billion in a new home and community-based long-term care program, the Dole bill does next to nothing for long- term care. Older Americans in need of assistance will continue to face no choice but to enter nursing homes. Discriminates Against Smail Companies With Older Workers Small firms with more than 50 employees will remain at the mercy of insurance companies who charge higher rates for older workers, higher rates for sicker workers, and raise rates when even one employee gets sick. This means older workers will have their jobs at risk when their employers look at their insurance premiums. One of the\nAdministration's biggest health care reform supporters, the AFL-CIO, has said it cannot\nsupport the Mitchell bill and that the bill should be strengthened to assure universal\ncoverage.\nThe AFL-CIO supports the Gephardt bill and so does the President. We agree and Senator Mitchell agrees that 95% is\nnot universal coverage and that's why he put mechanisms in his bill to go beyond and reach\nuniversal coverage.\nDoes the Administration think that the Mitchell bill needs to be strengthened?\nThe Administration wants universal coverage. That's our bottom line. We believe that the\nMitchell bill achieves universal coverage. If the Senate strengthens the Mitchell bill, that's\nfine. As the President said last night, he is not going to get into being a legislator. Congress\nwill make changes, that's part of the legislative process. Our bottom line is universal\ncoverage.\nSenator Dole says that his plan solves the problems of people like those at the press\nconference last night. Haven't you consistently said that you reject a large government-run\nprogram?\nThe House leadership has chosen to adopt the method similar to that developed in the\nWays and Means Committee bill as one of the ways to cover those who are not employed\nand as an option for small firms. It helps achieve universal coverage. and, like many other\nissues. will be the subject to continuing debate.\nAbortion is in both the House and Senate bills, but is not expected to pass as introduced.\nYou had abortion coverage in your bill. Will you sign a bill without abortion coverage?\nMost plans cover abortions today. My bill included pregnancy-related services, including\nabortions. Would you accept such a\nmandate?\nI have said all along that I would be flexible on how to get to universal coverage, but not\nwhether we get to universal coverage. Senator Mitchell has put forth legislation to bring us\nto universal coverage. Congressman Gephardt has put forth legislation to bring us to\nuniversal coverage. We have to see how the debate proceeds, but the bottom line is that it\nhas to bring us to universal coverage.\nDo you support the exemption for small business being put forth by Senator Mitchell?\nThe current health care system is stacked against small businesses. Many provide and\nthose that don't want to. I have said all along that I want to make sure that small\nbusinesses are protected - so that they can provide insurance and keep small businesses\nthriving.\nBut do you support such a concept?\nMy bottom line is universal coverage. I'm flexible on how you get there, but not whether\nyou get there.\nDo you have the votes for a mandate in the Senate?\nIt will be close, but we will prevail\nDo you have the votes now?\nThe bill was just introduced. It will be close. but we will prevail.\nThe American people want universal coverage. They understand that without it middle\nclass Americans will not have real health security. I think when the House and Senate\nengage in the health care debate on the floor, they will see that what we are talking about is\nwhether they turn their backs on hard working middle class Americans or not. I believe the\nAmerican people will be engaged in this debate and will be sending a strong message to\ntheir members of Congress that nothing less than universal coverage will be acceptable.\nIsn't it true that the House is afraid of being BTUed? If the mandate fails in the Senate,\nwon't that bring it down in the House as well?\nI think the Senate and House will both pass a bill with universal coverage. I understand the\nconcept of being BTUed, but I think members of Congress understand that a vote for\nuniversal coverage is a vote for the people that they represent. It's a vote for middle class\nAmericans who are one job loss or one illness away from losing their health care coverage.\nIt seems from this morning's papers that there is tension in the House given what is\nbeing introduced in the Senate. Won't this tension lead to a more moderate bill like that\nbeing proposed by Cooper?\nThis is a complicated issue and members of both Houses are trying to grasp all of the\ndetails of the two bills being put forth. I think as the debate proceeds and members focus\non the fight for health security for their constituent, for hard working middle class\nAmericans, the answer will be clear: we need universal coverage.\nThe Annenberg Public Policy Center said yesterday that the White House elevated the\neffectiveness of the Harry and Louise ads. Obviously, this kind of money is going\nto have an impact on the public. But the bottom line is that the American people are\ncommitted to the President's bottom line of universal coverage. It helps seniors get the\nhome and community-based care they need and helps cover the cost of extended nursing\nhome care.\nAARP Has Endorsed the Mitchell Bill\nThese important benefits of prescription drugs and long-term care are among the reasons that\nthe American Association of Retired Persons (AARP) recently endorsed the Mitchell proposal.\nThe AARP said that the Mitchell bill provides an \"historic opportunity to provide each of us\nwith affordable, high-quality health and long-term care.\"\nLimits How Much Insurance Companies Can Charge Seniors\nIn today's system -- and under haf-measures -- insurance companies can charge older people\nas much as four times as much as younger people. That will be illegal under the Mitchell bill.\nSo older Americans -- not just young, healthy Americans -- will have real, honest insurance\nthat you can depend on.\nSeniors Will Still Have Choice of Doctor\nMedicare will be protected, and older Americans will be able to choose their doctor /-- just as\nthey do today. People will be able to keep their doctor and receive all the benefits they get\nfrom Medicare today.\nThe Bob Dole \"You're Out Of Luck\" Plan\nIf you're looking for health insurance you can count on you're out of luck.\nUnlike Senator Mitchell's plan. which guarantees secure. affordable insurance. the Dole plan provides no\nguarantee of decent coverage. Millions would still have phony, fly-by-night insurance, and an estimated 30\nmillion Americans would have no coverage at all [Lewin-VHI, July 1994]\nIf you're looking for health insurance you can afford you're out of luck.\nThe Dole plan allows insurance companies to continue to raise rates higher and higher each year, and\nto charge older people three to four times more than younger people. Some small businesses will\ncontinue to pay more than others, and some families more than other families. You could still work\nhard, pay your premiums, and have medical bills sent back \"not covered\".\nAs his Republican Colleague John Danforth said of the Dole plan. \"It creates a new entitlement and\nit doesn't have any cost control I don't think we can do that. \"[AP, 8/10/94]\nNewsweek predicts the Dole plan \"will increase premiums for middle class people and could increase [the]\nnumber of uninsured.' [Newsweek, July 25. p. 19]\nIf you're a senior you're out of luck.\nThe Dole bill takes significant money out of Medicare and does little or nothing for seniors - no\nprescription drug coverage, and pitiful help with long-term care.\nIf you're a child with no insurance you're out of luck\nThe Dole plan will continue to leave an estimated 30 million people with no coverage, including 6.2\nmillion children. What does that mean? In means millions of kids will go without seeing the doctor,\nmillions will not get needed health care in time to prevent disabling illnesses. And under Dole's plan, even\nworkers who get coverage can find that their insurance policy covers them, but not their kids. Employers\nwill continue to drop back family coverage and offer bare-bones, worker-only policies -- leaving millions\nmore children at risk of losing the coverage they have now.\nIf you're a small business you're out of luck\nThe Dole bill continues to permit insurers to charge higher rates to small employers just because they're\nsmall [Dole Bill 9002(d)(1) p. 117] Small companies pay 35% more on average as it is, and this\ninsurance company abuse against smaller employers will mean the little guy still gets overcharged. And\nunlike the Mitchell proposal, there are no subsidies to help small businesses who can't afford coverage\ntoday afford it tomorrow.\nIf you lose your job you're out of luck\nThe Dole plan theoretically allows people to take their same insurance with them when they leave their job\nand go to a new one (portability), this only helps those who can pay the full premium themselves. That is\nnot realistic for most people, since they can't afford the full cost of coverage, especially if they are without\na job. In fact, most workers have that protection now, either through state insurance laws or through\nCOBRA coverage. But as a recent study by the Department of Labor shows, only one in five can take\nadvantage of it -- the rest can't afford it. Even Senator Chafee admits:' have great trouble seeing how you\nget portability without universal coverage.\"\nThe Bob Dole \"The Insurance Company Protection\" Plan\nInsurance Companies Can Still Deny Coverage Through Loopholes and Fine Print\nWhile the Dole plan describes a standard benefits package (called the FedMed package) and says that\nevery health plan has to offer that option. it also allows insurance companies to offer any benefits\npackage they want. [Dole Bill Sec. 21115 (a) p.85 I This allows insurance companies to effectively deny\ncoverage of certain illnesses by structuring benefits packages to not cover certain treatments. Millions\nof Americans will continue to-face the nightmare of insurance claims coming back: \"Sorry, not covered.\"\nFew People Will Benefit From the Community Rating Reforms\nMany of the insurance reforms in the Dole bill, modified community rating for example, apply only to\nthe \"community rated market\" but there is no \"community\" in the bill's reform. Any employer can\nself-insure, so employers with young, healthy employees will likely stay out of the community-rated\npool and provide insurance on their own. In addition. associations can opt out of the community-rated\npool and buy as a group, leaving even fewer people in the pool. This \"vicious spiral\" will lead to very\nhigh premiums in the community-rated market and will encourage healthier people to drop coverage.\nMid-Size Businesses Could Face Large Premium Increases\nUnder the Dole Bill, many small to medium-sized companies would be left between a rock and a hard\nplace -- too small to self-insure, but too large to get the benefits of insurance reform in the community-\nrated pool. An employer with 55 employees could have extremely high costs because of just one\nemployee with a history of illness. They could see their rates jacked up based on one worker with a\nserious medical condition. For most firms in this category, insurance costs will continue to be high and\nunpredictable.\nInsurance Companies Can Decide They Don't Want to Sell to Small Businesses\nInsurance companies can decide that they want to avoid the small business sector altogether and refuse\nto sell insurance to any small businesses. Under the Dole plan, they could continue to deny coverage to\nsmall businesses.\nInsurance Company Profits Protected, Middle Class Families Left Waiting\nThe Dole plan limits, but does not eliminate. the ability of insurance companies to deny\nor forestall coverage for \"pre-existing conditions\" [for six months to a year]. While all\nagree that pre-existing condition limitations are a necessary transition tool to universal\ncoverage, the Dole bill will never get there, so exclusions will never go away.\nInsurance Companies Could Charge You Three to Four Times More Because You're Older\nThe Dole plan allows insurance companies to charge older workers three to four times more for\ninsurance than younger workers, and twice as much as under Senator Mitchell's bill.\nAmericans with Insurance Would Still Be At Risk of Losing It\nThe real bottomline is that under the Dole plan. everyone remains at risk of losing the\ninsurance they have now -- because under a non-universal system, no one is guaranteed\nprotection. As Newsweek magazine reported last week, the Dole plan \"will increase premiums\nfor middle class people and could increase [the] number of uninsured.\" [Newsweek, July 25, p. 19]\nThe Bob Dole \"Cheating Seniors\" Plan\nRaids Medicare and Gives Seniors Nothing Back\nWhile other bills reduce the rate of growth in Medicare. the Dole Bill relies heavily on savings\nfrom the Medicare program to finance reform. The Dole bill gives nothing back to the older\nAmericans that Medicare was set up to serve. No help with prescription drugs. Nothing but lip\nservice on long-term care.\nForces Millions of Seniors to Choose Between Food and Medicine\nThe Dole plan does not add prescription drug coverage to Medicare -- giving millions of older\nAmericans no help paying for costly prescriptions. Prescription drug costs are the highest out-of-\npocket expense for three out of four seniors, and the Dole plan would provide no help, forcing\nmillions of older Americans to continue to choose between food and medicine. More than 31\nmillion Americans under age 65, and 18.5 million Americans over age 65 would be denied\nprescription drug coverage under the Dole bill. Older Americans in need of assistance will continue to face no choice but to\nenter nursing homes.\nDiscriminates Against Smail Companies With Older Workers\nSmall firms with more than 50 employees will remain at the mercy of insurance\ncompanies who charge higher rates for older workers, higher rates for sicker workers, and\nraise rates when even one employee gets sick.
