Mary- This should be far more legible than the first version you saw. There may be minor
changes yet, but we do not expect anything significant. There are a few spots in the
recommendations chapter in bold italics where I have made comments for the author's review.
Just ignore them. You can also ignore the formatting errors and minor editing, all of which
should be corrected as this gets put into the publishing software.
