Chief complaint: Pain in the right lower abdominal quad-
rant; intermittent gas pain throughout the right lower quad-
rant and up into the mid-abdomen, on the right side; pain,
with minimal pressure, over the mid ascending colon and
cecum. other complaints are pain over the bladder area;
frequent blood cells in the urine in the past week; burning
with urination for the past 7 days. Another frequent com-
plaint is moderately severe frontal retro-occular headache.
Headaches are especially severe on the right side radiating
through to the temporal and occipital area, particularly
severe after a hard day of reading or long day of reading.
Walking and rest seems to ease the headaches.
General history: He wears glasses with
deviation.
He must wear one set for hyperopia and one for myopia. He
has occasional dizziness; ringing in the ears; headaches
quite often from long reading. General history, otherwise
is essentially normal.
Family history: Mother died following a fracture of the
hip, age 95, and secondary infection; arteriosclerosis,
arteriosclerotic heart disease. Father died following an
injury, which he received when a tree fell on him. No
familial history of diabetes, T.B., cancer, heart disease,
epilepsy or blood dyscrasia.
Marital history: He is married and has one child, daughter.
Previous diseases: Measles, mumps, whooping cough, chicken-
pox.
He does not use tobacco in any form. Occasionally has an
alcoholic beverage for cardiac stimulation. Very rarely
ever drinks any coffee.
Former operations: Cholecystectomy and appendectomy.
PHYSICAL EXAMINATION: Reveals a well developed, well nour-
ished, rather obese male. Weighs 165 pounds. Height 5'9".
Blood pressure, 128/82. Pulse, 82. Temperature, 99.6°.
