Oct. 24 '94 :6:25 0000 AMEMB CAIRD TEL 011-202-356-4929 P.2 A Foreign Service spouse with considerable experience in moving, Wendy Walker has spent more than half her life abroad, mainly in the Middle East. Born in England and educated R.A.F. style at numerous bases, Mrs. Walker joined the British Foreign Office in 1967 as a secretary and was posted first to Dar as Salaam and then to Tel Aviv where she met Ned Walker. They married in Jerusalem in 1973 and then she came to the States for the first time that year. She became s naturalized American citizen in 1973. Evacuated from Beirut in 1975 she helped move the Foreign Service Institute Beirut school to Tunis in 1975 and then continued onto Cairo with Mr. Walker the same year to finish Arabic studies. After a follow-up tour in Damascus (1977-1979) the Walkers returned to the States to adopt two children (Katie, born in Colombia in 1981) and Christopher (born in Weshington in 1982). In 1984 the family moved to London for a year (Royal College of Defense Studies) and then went on to Riyadh (1985-1988) and Abu Dhabi (1989-1992). During these years Mrs. Walker. in addition to her duties as the wife of a diplomat, ran the small Foreign Buildings Office (FBO) in the Embassy during the construction of new housing, worked for a short time in an American law firm, helped with a research project at the Documentation and Research Center and volunteered at the American School during the Guif War. She also volunteered with a group visiting severely handicapped children and, during the war, encouraged the community to support the American troops passing through the area. In 1992, the family returned to the States and went to New York in January 1993 where Ambassador Walker was Deputy Permanent Representative of the United States to the United Nations. During this short tour Mrs. Walker worked in a law firm and chose to volunteer among the homeless at night. Just over a year later she packed the family up for Cairo. In Cairo Mrs. Walker is back to her Arabic studies while pursuing a Batchelor's degree in sociology through the University of Maryland overseas program. 700 0 NEVER 0911 9CL 2020 ST:05 10/24/84 Women in Egyptian Society Egyptian law provides for equality of the sexes, but aspects of the law and many traditional practices discriminate against women. Egyptian women have employment opportunities in government, medicine, law, academia, the arts, and, to a lesser degree, in business. About 100 officers in the Egyptian diplomatic service are women, including 3 ambassadors. Social pressure against pursuing a career remains strong, however, and some Egyptian feminists say that a resurgent Islamic fundamentalist trend limits further gains. Women's rights advocates also point to other discriminatory attitudes and practices such as female circumcision and the male relative's role in enforcing women's compliance with religiously prescribed codes of sexual conduct. Women generally do not hold positions of senior leadership in the Egyptian government or political parties. The Constitution reserves ten (out of 454) People's Assembly seats for presidential appointees, which assures some representation for women. Ten women hold Assembly seats: seven elected and three appointed. Laws affecting marriage and personal status generally correspond to an individual's religion, which for most Egyptians is Islam. A 1979 liberalization of the family status law strengthened a Muslim woman's rights to divorce and to child custody, but in 1985 the changes were found unconstitutional on grounds that they conflicted with Islamic law and were repealed. There are at least two active women's rights groups in Egypt, one affiliated with the Egyptian Organization for Human Rights. The other is the Communications Group for the Enhancement of the Status of Women, which has published a booklet on the legal rights of Egyptian women. HEALTH SECTOR PROFILE Health standards continue to be a serious problem in Egypt, resulting in reduced productivity, depressed quality of life and lower life expectancy. The four leading causes of death for Egyptians of all ages are chronic heart disease, lung disease, infectious diseases and accidents. Debilitating parasitic (schistosomiasis), bacterial (typhoid, TB) and viral (hepatitis) diseases are endemic among the adult population. Maternal deaths due to childbirth are still high at 184 per 100,000 live births. Schistosomiasis is an important, but steadily declining, cause of morbidity for all age groups. Waterborne gastrointestinal diseases strike all age groups. Prior to the national oral rehydration program, 50% of all deaths were of children under age five, and half of those were due to diarrhea. Acute respiratory infections now rank first among causes of child death. Malnutrition has been greatly reduced through the COE's subsidy of basic foodstuffs, but there is still significant maternal and child malnutrition due to iron deficiency, poor weaning practices and intestinal disease. The GOE has long given high priority to the provision of public health services. Over 3,200 clinics, 314 community hospitals, and about 50 modern urban hospitals give most Egyptians access to some sort of health care. The doctor to population ratio is among the best in the developing world. However, poor quality of the public health care system. especially in primary Care, results in under Utilization of public health facilities. The GOE acknowledges that its health services must be improved and is facilitating change. Preventive health programs, particularly child survival programs such as immunization and oral rehydration therapy (ORT) are gaining priority but are still underfunded. A very nigh proportion of public health resources support expensive curative care efforts. The Ministry of Health plans to introduce cost recovery mechanisms and to shift more curative health services to the private sector. The current five-year nealth plan (1992-1997), emphasizes the importance of preventive health care but budgetary allocations favor hospital services. More aggressive programs in reproductive health and family planning, prevention of micronutrient deficiencies and other interventions that protect maternal and child health are needed. USAID's projects in this sector support preventive care, including immunizations, treatment of acute respiratory infections, and child spacing. In order to ensure the sustainability of improved health care, USAID is helping the GOE establish a rational financing system for health services, emphasizing cost recovery for curative CATA, greater GOE allocations for naternal and child health programs, and greater participation of the private sector in the provision of all health services. EDUCATION SECTOR PROFILE Egypt's educational system docs not contribute as much as it should to rapid economic growth. Nor does it allow the majority of Egyptians to atain their full human potential. Some of the problems Egypt faces in this secius are: inadequate physical facilities; increasing enrollments without a commensurate increase in resources devoted to the educational system: inadequate access to schools, especially in rural areas; limited equipment and books available to students at all levels; and poor quality of instruction. This contributes to a high primary school dropout rate and poor learning at all levels. When USAID began project assistance in this sector, first grade enrollment was below 83 percent In his first speech after re-election in October 1987, President Mubarak identified educational reform as one of the four areas requiring major national efforts (the other three being health. housing and tourism). The Ministry of Education has embarked on a major effort to formulate a strategy for educational reform. The strategy is designed to provide sufficient funds for school buildings w reduce crowding, to enhance technical education at secondary and higher educational levels and to upgrade and main teachers. USAID has provided $375 million of education and training since 1975. Lu a major effort to increase primary school enrollments, USAID Is building 1,800 schools in. rural areas currently underserved (e.g. the nearest school is at least five kilomaers away). Over 1,000 schools have been completed and new schools are being built at a rate of one per day. The impact on primary school enrollments is striking. In the three years since the first schools were completed, boys' first grade enrollment in those carchment areas increased 15 percent. Girls' first grade enrollment increased much more - 29 percent. Dropout rates in grades two through six decreased at a rate of six percent for boys and 17 percent for girls. Dropout rates are reduced markedly as schools are built nearer to where children live. Since 1975, USAID has financed over 8,500 participants in academic and technical training in the U.S. USAID's contribution to university education and research has been through the linking of U.S. and Egyptian universities for developmental research activities. The projects in this sector, and in the Science and Technology and Agriculture sectors, have helped develop relationships between U.S. and Egyptian scientists that strengthen the capabilities of Egyptian research institutions to engage in applied research. The standard Egyptian medical school curriculum provides little exposure to patients or patient care, and none at all to primary health care services. In a significant break with tradition, the Suez Canal Area Medical Education and Health Services project has developed a six-year course of studies in the Sues. Canal University Faculty of Medicine that provides greater student exposure to local health needs. First year students are placed in rural and urban health facilities. Patient health problems serve as basic learning units. As the students progress, they receive greater patient exposure and, ultimately, responsibility for diagnosing and recommending patient treatment. Out of 50 students who entered in the first year, 44 students or close to 90 percent graduated in 1988. Forry-five percent of the students are female. A. POPULATION SECTOR PROFILE Despite dramatic gains in the use of family planning, Egypt's annual population growth rate, reported by the GOR to be 2.3 percent in 1991, remains one of the central constraints to the country's economic growth. The negative effects of high fertility are evident in many aspects of Egyptian life: population distribution, education, employment and health. Acknowledging that rapid population growth rates jeopardize all of Egypt's development efforts, the GOE currently invests substantially in family planning services. The President and influential religious leaders like the Grand Mufti and the Pope of the Coptic Church frequently call attention to the importance of family planning. Since 1975, USAID has been the largest single donor to Egypt's family planning program providing about 75% of all donor assistance. As such, AID's strategy is to assist the GOE in establishing effective population policies; assure an adequate supply of contraceptives and related supplies; improve access to family planning services and information through the public and private sectors; improve the quality of service delivery and contraceptive use; and develop program coordination, management and information systems. USAID has spent approximately $170 million to establish an institutional framework for family planning, train Egyptian health personnel in family planning, promote and distribute contraceptive methods, provide the neoded research base (both operations and applied biomedical), and to make quality services more available and accessible. In 1987, project efforts were reviewed and revised to put priority on the delivery of quality family planning services which are acccosible and emphasize longer acting methods. Documentary evidence from recent evaluations suggests that the positive achievements of the program are directly associated with this strategic emphasis. With these successes in expanding population and family planning programs, a new set of implementation issues arose. The Population/Family Planning III Project addresses these issues by building on the successes of Population/Family Planning II and by utilizing a more focused and consolidated approach. The conference recommended that all laws be revised to "remove any reference to man-woman distinctions" but added that this should be done "within the framework of our religious and ethical values cultural heritage and political, economic and social conditions". The conference made particular mention of the personal status law. It said some articles of this law "should be studied objectively to safeguard the rights of women". The provisions of marriage and divorce contracts should be revisedand the litigation process should be made easier to make it possible for women to regain their rights "within a reasonable period of time", the recommendation said. Another recommendation emphasised the importance of women's participation in politics. This, it said, should be accomplished by urging women to register themselves as voters and by devoting greater attention to their political education. Combating women's illiteracy was also the subject of attention. The recommendations said that illiteracy should be viewed as a "principal challenge facing Egyptian women and that effective methods and a time-plan should be devise to eradicate illiteracy within the coming years". The conference declared 16 March of each year as "Egyptian Woman's Day". On that day in 1919, Egyptian women staged their first demonstration in modern history to protest the continued British occupation of Egypt. BACKGROUND ON ISRAELI AND JORDANIAN HEALTH CARE SYSTEMS In Israel, expenditure on health care is about 8% of GNP; the average annual per capita expenditure on health care of $372 is much lower than in most developed countries. Israel's two main providers of health care are the Ministry of Health and Kupat Holim or general sick fund. The General Federation of Labor Unions (Histadrut) management of Kupat Holim led to financial crisis in 1992, and despite some steps to control costs, the fund's total debt by 1994 stood at $1.3 billion. Histadrut has successfully opposed government efforts to sever the link between Histadrut finances and those of Kupat Holim; government bailouts and a new national health insurance law (that would generate up to $330 million) still leave the fund with a large operating deficit. Kupalit Holim covers 80% of the population and provides prepaid medical hospital, drug, and other services. The Ministry of Health provides most preventive components of primary health care, which are available to all, regardless of health insurance status. Approximately 25% of services are funded directly by the government; 15% is funded by the public through prepaid voluntary insurance plans, and 30% from employers' obligatory contributions on behalf of their employees; the remainder is received from payments for private services and from various non-profit organizations. Major complaints about the system focus on over-bureaucratization of health care. The recently established National Health Council is designed to be a forum for interagency exchange of views and discussion on health needs and priority programs at national level. There is still a lack of coordination between services and in the comprehensiveness of services, especially at the regional and primary care levels. The quality of health care in Jordan has improved over the last two decades and is now among the best in the developing world, in many cases approaching levels in industrialized states. Jordan dramatically reduced infant mortality, raising life expectancy, improved rural health services and achieved almost universal access to clean water. However the influx after the Gulf War of almost 300,000 returnees imposed a severe burden on the country's health care facilities. The government's major challenges include coping with funding shortfalls in the face of increased demand and lack of coordination among public institutions that is raising costs and lowering the quality of care. The government is working on legislation to provide health insurance to all Jordanian citizens and improve basic health services. In 1990, public spending on health services amounted to 6% of GNP. Jordan has one of the highest population growth rates in the world (3.4%); consequently, both Jordanian government and U.S. AID programs focus heavily on population planning and primary health care. Israel's two main providers of health care are the Ministry of Health and Kupat Holim or general sick fund. The General Federation of Labor Unions (Histadrut) management of Kupat Holim led to financial crisis in 1992, and despite some steps to control costs, the fund's total debt by 1994 stood at $1.3 billion. Histadrut has successfully opposed government efforts to sever the link between Histadrut finances and those of Kupat Holim; government bailouts and a new national health insurance law (that would generate up to $330 million) still leave the fund with a large operating deficit. Kupalit Holim covers 80% of the population and provides prepaid medical hospital, drug, and other services. The Ministry of Health provides most preventive components of primary health care, which are available to all, regardless of health insurance status. Approximately 25% of services is funded directly by the government; 15% is funded by the public through prepaid voluntary insurance plans, and 30% from employers' obligatory contributions on behalf of their employees; the remainder is received from payments for private services and from various non-profit organizations. Major complaints about the system focus on over-bureaucratization of health care. The recently established National Health Council is designed to be a forum for interagency exchange of views and discussion on health needs and priority programs at national level.
