Subject
Referring to today's conversation, I em handing you herewith, application
for passport, form of affidavit required by the War Department to be ececuted
in triplicate, and card of the Brooks Studio whore you can have your passport
photos taken at any time suitable to your own convenience.
You will also require a birth certificate, or in lieu thereof, an old
passport which has been issued to you,or an affidavit by some person who is
competent to swear to the fact of your birth in the United States.
When the passport application is complete, it will be necessary for you
to go to the Bureau of Citizenship at 1433 New York Avenue, accompanied by a
resident of Washington who has know you for at least two years, who will make
a supplementary affidavit of identification.
I understand that you have handed Mr. Millett the usual form of application
for Red Cross service and with this wé will obtain the necessary War Department
clearance.
