EVALUATION FORM FOR RED CROSS HOME NURSING INSTRUCTORS
(Three copies of this form should be sent to the area office and
one retained by the rater for her own files. See ARC 1605.)

II. CHAPTER NAME AND ADDRESS Nursing Field Representative, New Jersey, North Atlantic Area
III. PRACTICES IN TEACHING (points to consider)
Is the instructor familiar with the subject matter; is the course planned on
the basis of individual and group needs and interests; how were the course
objectives for the group selected; are teaching aids such as posters, pamphlets,
and references scientifically true and directly related to the discussion or used
without direction; are equipment and classroom adequate; do students partici-
pate in discussion and are they interested, or does telling and lecturing by
the instructor predominate; if students are not interested, why not; what
methods are used to develop interest; how are individual interests, experience,
and background utilized for the benefit of the group; are assignments
planned on the basis of individual and group needs; what type of tests and
other methods of evaluation are used?
Attended North Atlantic Area Master Conference at New York Chapter, July 13 to July 27,44
Recommend that Mrs. Heilman be authorized to teach "Six Lessons in Care of the Sick"
Mrs. Heilman is familiar with the subject matter but has some reservations on accepting
all details of the procedures. She is, however, very much in favor of the method of
teaching.
IV. PERSONAL CHARACTERISTICS (appearance, manner, diction)
Very nice appearance, pleasing manner, and excellent diction.
