Dir., TITLE Nursing Service INSTRUCTIONS COVERING PREPARATION (1) Covers only travel and maintenance from home to place employed or vice versa. If worker is to be reimbursed regularly for travel and maintenance this should be indicated in space provided. Copies shall be made as follows: At National Headquarters: 1 copy for salaries less than $5,000; 2 copies for salaries of $5,000 or more. At Area Offices: (a) Less than $5,000, 2 copies shall be made and forwarded to National Headquarters. (b) $5,000 or over, 3 copies shall be made and forwarded to National Headquarters. In case of personnel transfers with an area or at National Headquarters, the responsibility for initiating the pay roll changes shall rest upon the Director of Personnel. Salary rate shall always be shown except in the case of a release or resignation. The name of the appropriation shall be shown as well as the appropriation symbol. When temporary personnel are recruited and rassigned by the manager of an area to a disaster field operation, three copies shall be pre- pared. See also Chapter XI, "Personnel," Disaster Relief Handbook (ARC 234) covering use of this form for disaster relief personnel. 9
