You have been scheduled for your blood count and
expired air excumination on Wednesday, December 8th. You are to report
at 9:00 A.M. to:

New York City
Above location is at 50th Strest and Amsterdam Avenue. Take 8th
Avenue Subway to 59th Street and walk two blocks toward river.
Wash your hair the night before the exenination,
bathe and wear all clean clothes. Also bring with you a specimen
of urine, collected early A.M. of day of examination.
If, for any reeson, you cannot go, please notify
us immediately so that other arrangements can be made.
