child; (2) Physical or mental incapacity (if applicable); (3) Attending (a job training or
educational program); (4) Job training and educational program; (5) Residing with; (6)

considered in protective services for purposes of child care eligibility; and whether respite
care is provided to custodial parents of children in protective services; (8) Very low income;
