As a part of our study of radium protection, we need to
have some fairly accurate information regarding the sale and
distribution of radium and radium salts. Your cooperation will
be appreciated if you could give reply to the following questions.
If any of your replies are confidential or if you are disinclined
to answer any of the questions, will you please so indicate?
1. Amount of American radium element sold by your concern
per year?
2. Amount of foreign?
3. Names of foreign sources and approximate quantities re-
ceived for each?
4. Amount of radium element per year sold to medical profession?
5. Amount per year to watch manufacturers? (Give list of the
6. Amount per year to other manufacturers?

7. In what form is it usually sent to manufacturers?
8. Names of any other American distributors of radium?
9. Is all redium sold by your company certified by the Bureau
of Standards? If not, give particulars.
10. Number of deaths or accidents directly traceable to radium
poisoning in your company?
11. Any other information which you may feel would be of as-
sistance in giving us a synopsis of the radium distribu-
tion in this country.
