CLAIM DEPARTMENT
THE PRESENT METHOD OF PROCEDURE AS INDICATED
1.
All report blanks known as C-2 forms are to be typed in triplicate,
two to be mailed to the State Insurance Fund, one to be kept in
assured's file.

Accidents which occur in the course of employment are to be reported
by the injured employee at the time of occurrence so that the employee
may receive proper medical relief. A C-11 form is to be filled out
when employee returns to work.

There have been fifteen new amendments to the compensation law which
became effective on July 1, 1935 and in particular Sec. 13 A-l, which
gives the primary right to the employee to choose his own doctor provided
he chooses a doctor from the paneled list.

The waiting period has been reduced from forty-nine days to thirty-five
days, so that if an employee is disabled for more than thirty-five days
he is entitled to the first seven days, but only then. If period of dis-
ability is less than thirty-five days, the employee will receive compen-
sation every day after the seventh day, but not the seventh day, up to
the time the doctor treating him discharges him from medical treatment.
Benefits amount to two=thirds of average weekly earnings not to exceed
$25.00 per week.
5. At the time of occurrence, the employer can send the employee to the
nearest doctor or hospital for emergency treatment. If subsequent
treatments are required, it is up to the employee to choose his own
doctor, provided the said doctor is on the paneled list.
